Good Faith Estimates

You have the right to receive a “good faith estimate” explaining how much your medical care will cost:

  • The law requires health care providers to provide patients with a Good Faith Estimate for billed services who do not have insurance, or who are not using insurance.
  • CASH PAY PATIENTS: Wellspring Physical Therapy will provide you with cost per visit information during the intake process, payment will be due at time of service.
  • PATIENTS BILLING INSURANCE: Wellspring Physical Therapy will provide you with an insurance verification reviewing an estimate of patient responsibility, this information is obtained from your insurance provider. Patient responsibility of co-pays, estimated co-insurance and $100 towards high-deductible plans will be collected at time of service; remaining patient financial responsibility will be collected at the clinic as insurance reimbursements are processed.
  • You may ask your provider for an estimate before you schedule services.
  •  If you receive a bill that is at least $400 more than your good faith estimate, you can dispute the bill.

First Visit

​What to Bring

  • Insurance card and photo ID
  • Referral from doctor, if required by your insurance provider
  • Results from recent imagining, such as x-ray or MRI
  • If being seen after surgery, bring the surgical report and MD protocol for physical therapy

Patient Intake Paperwork

Please complete your Patient Intake Paperwork before you arrive or arrive 20 – 30 minutes before your initial evaluation appointment time to complete it. We will attach the forms to your welcome email. Or, you can download the forms from the Patient Center section above.

What to Expect

You will receive a forty-five-minute evaluation, during which your licensed provider will:

  • Review your injury history
  • Assess your current status and injury
  • Establish a home exercise program
  • Establish an active plan of care
  • Answer your questions and concerns

What to Wear

Wear comfortable clothing. If you are seeking treatment on a hidden body part such as your shoulder or knee, loose and stretchy clothing works best. Wear comfortable shoes for walking or standing activities.

Health Safety Practices

The health of our patients and staff is our number one priority. To ensure your safety, we have taken several measures to maintain a clean treatment environment:

  • Private treatment rooms. A private treatment room is available to each patient.
  • Air filtration and disinfection. Every treatment room has a HEPA filter with UVC disinfection.
  • Limited contact. Each patient receives 1:1 treatment from a single provider.
  • Sanitizing. We clean rooms and equipment between patients.
  • Masks are provided if needed.
