First Visit

​What to Bring

  • Insurance card and photo ID
  • Referral from doctor, if required by your insurance provider
  • Results from recent imagining, such as x-ray or MRI
  • If being seen after surgery, bring the surgical report and MD protocol for physical therapy

Patient Intake Paperwork

Please complete your Patient Intake Paperwork before you arrive or arrive 20 – 30 minutes before your initial evaluation appointment time to complete it. We will attach the forms to your welcome email. Or, you can download the forms from the Patient Center section above.

What to Expect

You will receive a forty-five-minute evaluation, during which your licensed provider will:

  • Review your injury history
  • Assess your current status and injury
  • Establish a home exercise program
  • Establish an active plan of care
  • Answer your questions and concerns

What to Wear

Wear comfortable clothing. If you are seeking treatment on a hidden body part such as your shoulder or knee, loose and stretchy clothing works best. Wear comfortable shoes for walking or standing activities.

Many insurance plans have limits on therapy services, either dollar or visit limits. COVID Safety Practices

The health of our patients and staff is our number one priority. To ensure your safety, we have taken several measures to minimize the possibility of COVID exposure.

  • Private treatment rooms. Each patient is treated in a private treatment room
  • Air filtration and disinfection. Every treatment room has a HEPA filter with UVC disinfection
  • Limited contact. Each patient receives 1:1 treatment from one physical therapist (no aides)
  • Sanitizing. We sanitize rooms and equipment between patients
  • COVID screening. Every person who enters the clinic receives a COVID screening, including a temperature check
  • Masks. Everyone in the clinic wears a mask all the time; masks are provided if needed
